Voluntary self-identification of disability

Ohio State is committed to non-discrimination, equal opportunity and affirmative action, and the university adheres to the laws and regulations that govern these areas.

As an employer that does business with the federal government, the university is required to measure its progress toward having at least 7% of its workforce consist of qualified individuals with disabilities. To do this, Ohio State must periodically ask employees to voluntarily update their disability status, which is the reason for this letter.

For these purposes, individuals are considered to have a disability if they have a physical or mental impairment or medical condition that substantially limits a major life activity, or if they have a history or record of such an impairment or medical condition. Some examples include autism, cancer, cerebral palsy, diabetes, gastrointestinal disorders, and nervous system and psychiatric conditions. Additional information is available on the voluntary self-identifcation of disability page on the Office of Institutional Equity website.

To update your information, please follow these steps:

  1. Log in at workday.osu.edu
  2. Access your profile using the profile menu (Click the cloud icon at the top right of the page)
  3. Click "View Profile"
  4. Click the “Actions” button below your name
  5. Expand the “Personal Data” menu, then click on “Change Self-Identification of Disability”
  6. Update the form as needed and click “Submit”
  7. If you need assistance accessing and completing this form or arranging any other accommodations, contact the ADA Coordinator’s office at ada-osu@osu.edu or 614-292-6207

The decision to self-identify a disability is strictly voluntary, and there are no consequences for not self-identifying. The university treats all individual demographic information as confidential to the extent allowed by law and maintains the data separately from personnel files.
